Summary:
Inservice Education/Training: Continuing onward from the start date of employment all employees within Twin Tier Home Health are required to attend a certain amount of Inservice Education/ Training. For Field Staff this requirement includes a minimum of 12 hour per year including any mandated programs. For Office Staff there is required attendance at The Annual Mandatory Employee Health and Safety Meeting.
CPR Re-Certification: For field staff only, with the exclusion of home health aides with eligibility from start date there is free training every two years. It will remain the employee's responsibility to be certified within the first 3 months of employment.
NYS Disability: Employees of Twin Tier Home Health are eligible for New York State Disability having been disabled for more than 7 calendar days due to a non-work related illness or injury. This illness and/or injury must be confirmed be a physician. For all employees, the eligibility for this Disability will be enacted one month from the start date of employment.
Family Medical Leave Act(FMLA): For all employees having worked 12 months with a minimum of 1,250 hours during that time period there is an eligibility for Unpaid leave time for up to 12 work weekes during any 12 month period.
United Health Direct Exclusive, United Health Preferred, MVP, Blue Cross/Blue Shield/Blue Point:
Dental: The first day of the month following when the employee has completed 30 days of service they are eligible to participate in one of two Dental Plans; the standard and the high option plan.
Long Term Disability: The first day of the month following when the employee has completed 30 days (with an average of a 20 hour work week) of service they are eligible for Long Term Disability
Bereavement: Full time and Part time employees of Twin Tier Home Health are eligible for Bereavement benefits immediately following their start day with this organization. A paid absence of a maximum of 3 days scheduled to work for the death of a member of the Immediate Family including: Father, father-in-law, mother, mother-in-law, brother, sister, spouse, or child. A one-day paid absence for the funeral, if scheduled to work, is given for the death of a Grandparent, Grandchild, brother-in-law, sister-in-law.
Malpractice Insurance: All licensed Employees within Twin Tier Home Health, starting on date of employment, receive the benefit of Malpractice Insurance. This covers bother employee's liability while performing duties within the scope of employment as well as the Agency's Liability.
Life Insurance, Accidental Death & Dismemberment: Employees are eligible to receive $10,00 in Life Insurance as well as $10,000 of AD & D if they meet the specified requirements. This eligibility starts the first day of the month following the month in which the employee completes 30 days of service. Employees of Twin Tier must also have worked a minimum of 17.5 hours on average. Additional life insurance is available for employees, spouse, and children.
Employees Choice Plan: PT employees with an average of 20-30 hours worked and FT employees with an average of 36-40 hours worked are eligible for dependent and health care reimbursement accounts following the first day of the month after the month in which the employees completes 30 days of service.
Child Health Plus: All employees of Twin Tier are eligible for this NY State Insurance Program. It covers children up to the age of 19 who live in NYS. This program is based on gross family income and there is monthly open enrollment for those who wish to benefit from this.
EAP: All employees are eligible for the Employee Assistant Program immediately following their start date.
Retirement Plan 403B: On the first day of the month following Date of hire employees with a minimum of 20 hour worked are eligible for the Tax Deferred Amnity Savings Plan.

Paid Time Off (PTO): Both FT and PT employees are eligible for Paid Time off Starting 30 days from the date of hire.
Direct Deposit: There is a two-week enrollment policy on this benefit. It is open to all employees and net pay will automatically be deposited in most participating banks.
Mileage: Following the start date both FT and PT employees are eligible to receive $0.33 per mile for their travels between office and patient homes and between two different patient's homes.
Cancelled Patient Visits: For all Field employees that are paid by visit there will be $10/Visit paid if one makes an appointment with a patient who then refuses or is not home when you make the visit.
Tuition Reimbursement Program: For Full time employees of Twin Tier Home Health that have one year of service there is eligibility for the Tuition Reimbursement Program. This will be offered for courses through an accredited institution that are directly related to your current position or a related career within Twin Tier Home Health.
Voluntary Insurance Program Services(VIP): There is monthly open enrollment for this services open to all employees. Employees are responsible for all costs under this program including Auto and Home Owners. In addition there are group rates and discounts.
Child Care Resources and Referral Service: This benefit is in cooperation with the Broome County Child Development Council and is open to all employees of Twin Tier Home Health.
Vision Program: All employees are eligible for this benefit upon start date. This provides savings of up to 40% for varied services at any Empire Vision
Centers. ID cards are available at your HR office.
