General Information
Following is a summary of typical employee benefits at Delaware Valley Hospital.
Specifics regarding each benefit are detailed in an employee benefits summary
and the insurance plan booklets and given to all new employees.
Medical: Employees can choose from two Health
Maintenance Organizations (HMO) plans. DVH pays the major portion of whichever
health plan the employee chooses: individual, 2 person or family coverage.
Coverage is effective the first of the month following 30 days of employment.
Dental: Coverage is effective the first of the month following 30
days of employment. The plan provides coverage for Preventative, Basic & Major
Services.
There is a modest payroll deduction for this plan.
The Plan Pays 100% of the usual cost of preventive services with no deductible
required. After a deductible of $50 ($150 deductible per family) each calendar
year, the plan pays 80% of the usual cost of basic services and 50% for major
services.
Vision: Coverage is effective the first of the month following 30
days of employment. The plan provides coverage for eye examinations, frames,
lenses, or contacts. There is a modest payroll deduction for this plan.
Group Life: Employees are insured for an amount equal to their
annual salary. DVH pays the full premium. Employees may also purchase
supplemental life coverage for themselves and their families at the DVH group
rate.
Short-Term Disability: In accordance with New
York State law, employees receive benefits equal to one-half their average
weekly earnings (up to a maximum of $170 a week) for as long as 26 weeks, after
a waiting period of seven calendar days, for absences caused by illness or
injury not related to employment. Employees contribute up to a maximum of $1.20
per pay period.
DVH Disability: Eligible employees accrue DVH
DBL each pay period as a means of compensation for time lost from work due to
illness or injury that does not arise out of, or in the course of employment.
DVH DBL will commence on the sixth (6) consecutive work day of disability. This
benefit is coordinated with NYSDBL to pay no more than full pay respectively to
an employee.
Paid Time Off: Employees earn paid time off
(PTO) for vacations, holidays, unexpected illness or absences, and emergency
personal days. PTO hours are earned each pay period. Scheduled PTO may begin any
time after employment. Unscheduled PTO may be used upon completion of the
six-month probationary period.
Bereavement Leave: A paid absence for up to
three days is available in the event of a death of a member of the immediate
family, after employees have successfully completed their probationary period.
Flexible Spending Accounts: Pre-tax
contributions to a flexible spending account for health care may be used for
reimbursement for medically necessary medical, dental, and vision expenses not
covered by health/dental insurance. Dependent Care account is available for
pre-tax reimbursement of dependent care expenses.
Workers Compensation: Employees who are absent
more than seven consecutive days because of work-related injury or illness will
be paid Workers’ Compensation benefits in accordance with New York State law.
Social Security: UHS Hospitals deducts FICA
Taxes from employee paychecks each pay period to be put into Social Security
System. FICA Taxes are also matched dollar for dollar, and these payments are
credited to the employee’s Social Security Account.
Credit Union: The Employee’s Federal Credit Union offers employees an
opportunity, through convenient payroll deductions, to build a personal savings
program or take advantage of low cost loans.
403(b) Retirement: Eligible employees may
contribute to the hospital sponsored tax deferred retirement savings plan with
Diversified. The Employer match begins the 1st of the quarter after 1 year of
service. The Employer match is 75% of the first 4% of salary deferral.
Leave of Absence: This is designed to provide
employees with necessary extended time off under appropriate circumstances.
Leaves traditionally are available for family and personal reasons, but may
include other reasons. Employees on an approved Leave of Absence may continue
certain benefits depending on the reason for the Leave of Absence.
Tuition Reimbursement: DVH reimburse employees
up to a maximum of $1,260 per year for successfully completing courses directly
related to their position or related career within DVH. Employees are eligible
for approval to enroll in courses after 6 months of employment.
Employee Assistance Program (EAP): Confidential
counseling and referral services are available at no cost to employees and their
families. Fees for services provided outside the EAP are the employee’s
responsibility but may be covered by the insurance plans.
Malpractice Insurance: UHS Hospitals provides
all employees with Processional Liability (Malpractice) insurance coverage and
pays the full premium for this coverage. This covers any activities performed
within the scope of their job description.
